 # The candidates signature  

# The candidates recent passport size photograph  

# The percentage mentioned in the application should be the

total aggregate percentage and not the percentage of only Maths or Science.

# If you have ticked any of the categories other than GM, photocopies of the relevant certificates should be enclosed along with the application. If the application is not having the relevant certificates (The Validity of the Category certificate should not be older than 5years), that APPLICATION WILL BE CONSIDERED AS GENERAL MERIT CATEGORY. No subsequent requests or changes to categories will be entertained once the application is submitted.

 # The provisional marks card attached with the application and

having the seal and signature of the school authorities.
